How many grams is one serving?
It varies by food. This calculator uses common serving standards such as about 210g for a bowl of rice, about 120g for one pack of ramen (noodles plus soup base), and about 90g for one piece of chicken. Actual portions can differ from home to home and restaurant to restaurant.
How are carbs, protein, and fat worked out?
They are calculated by multiplying each food's average carbs, protein, and fat per 100g by the amount you ate. Carbs and protein are converted at 4 kcal per gram and fat at 9 kcal per gram, and rounding may cause a slight difference from the total calories.
